Is HOA architectural change form needed?

Question:

We, as the board of our Florida association, want to do some cosmetic work to the outside of one of the structures...  mostly painting a differently color, adding wood based trim and adding three windows.  Should we be using and filing our own architectural change form so we have it?  Is it required?
