Looking for sample HOA or condo association violation letters

Our HOA has begun sending violation letters to residents. Is there any form of written procedure and samples of letters?



Our condo issues parking tickets ---but never fines. Noise violations are handled on an individual basis--such as "A neighbor has complained about hearing your high heels click when you walk on the Mexican Tile in your unit. Kindly place carpeting or remove your heels when walking on your tiles. We have not fined for that either. I imagine that if a repeat offender came along we might use fines. The most popular committee is the "appeals committee," because it never has to meet.

Under Florida Laws there are very specific requirements which must be followed if the Association intends to fine. 
I can consult on the issue to make sure you understand the requirements and provide a form letter for a small fee.

Does anyone have a sample waiver form so communication can be by email and waived by mail? Many of our homeowners are from out of the country so email would be preferable but statutes require a waiver.
